Welcome to Millbrook Central School District's Online Registration
Our online registration will guide you through the entire enrollment process for Pre K-12.
Here you will enter all of the student information and upload all of the supporting documents.
The Registration Affidavit, Free and Reduced Lunch Application and the Enrollment/Residency Questionnaire cannot be completed electronically. Please download and print pre-populated forms to sign and upload.
The NYS School Health Examination Form requires handwritten signatures and cannot be completed electronically. Please download and print forms to complete and upload them with the other supporting documents.
The NYS Migrant Education Program: Please download and print pre-populated Migrant Parent Survey to sign and upload, if applicable.
**Please note:
Children must be four years old on or before December 1st to enroll them for Pre K for the following school year.
Children must be five years old on or before December 1st to enroll them for kindergarten for the following school
year.
