Your child will NOT be enrolled 

until all necessary documents are submitted and verified by the Registrar****

There are 3 sections to be completed.

Step 1: Complete the online pre-registration.  To get started, please click "new user" on the left hand side.                 Please complete all 3 sections.

• Section 1 includes all demographic and emergency contact information. 
• Section 2 includes information about your student's history, housing and migrant status.
• Section 3 NYS Digital Survey that is currently required by the State.   

***Please complete each section and save session, it is not necessary to print the registration.*****

Step 2: Please call the registration office to setup an appointment.

****Please prepare to bring ALL necessary documents to your appointment.  See below for all that apply.****  

REQUIRED DOCUMENTS:

• Proof of Age - birth certificate, passport, baptismal certificate, etc.
• Parent/Guardian Photo ID
• Immunization records and a current physical. Physical is not required at the time of registration, only if available.
• 2 Proofs of Residency-- A lease is required or 2 other forms of residency.  If you are living with others, please submit the notarized affidavit along with 2 forms of residency from the family you are staying with. 
• Custody Papers, if applicable.
• Foster Students: case manager information and paperwork from DFS.

Other documents, if available but not required:

• Last report card/transcript
• Most recent IEP, if applicable
