Welcome to the Monticello CSD Online Pre-Registration Portal **This is only a pre-registration. Your child will NOT be officially enrolled until ALL required documents have been turned in and verified by the Registrar**
Completing the online pre-registration is the first step in getting your child(ren) enrolled in this district. To get started, click the "New User" option on the left. Once all of your information has been entered, submit the pre-registration session and note your confirmation details. Please note there are 3 sections to the online pre-registration; Pre-registration Form 1, Pre-registration Questionnaire, and NYS Digital Access Survey, be sure to click on each box and complete all sections. Please bring confirmation details, and completed health and language questionnaire along with the following to your appointment: REQUIRED DOCUMENTS: Birth Certificate Immunization Records Parent Photo ID Proof of Residency Custody Papers (if applicable) Registration is located in the St. John's Street Building 22 St. John's Street, Monticello NY 12701 The entrance is in the front of the building, to the far right, closest to Broadway.
